Hi, am 38 yrs old and from the uk,found this site while looking for places to get a reversal.Everything i’ve read seems really positive.I’m filling in the medical release form for the hospital and just wondered if the social security number that it asks for is our NHS medical card number? karen xx

Karen–I am glad you are considering Chapel Hill for your Tubal Reversal. We have had many patients from the UK and would welcome you. Let us know if we can help in any way. I believe the NHS card number would identify your chart for release of information.

Hi Kazra,
i am also from the UK your social security number is your national insurance number,but you will need your NHS number to get info on your medical backgroud, i went to my gp who emailed my info to me you could try that.

I am from the UK and had my TR in January gone by.
As Sarah said you will need your NHS number which your GP surgery will help you with for details. If you have any questions just ask and we will help as much as we can. Debs keeps on asking and we have never got stuck yet. Good luck on your decision.

thanks everyone for your replies. I’ve spoke to a lady in the med records dept at the hospital that i had the surgery done and she said to just post the med release form filled in to them and they will send my records off to Dr Berger. I hope i put the right number on the form, i put my NHS medical number on it. I could always phone chapel hill with my national insurance number if they need it. I’m feeling quite positive about things now, I’ve had a go at IVF and i got pregnant but miscarried at 7wks, then had 4 frozen embryo replacements all of which came out negative. I’m hoping that TR will be the answer, me and my partner want a baby together so much. Karen xx

If you dont mind me asking! what part of the UK are you from?
i’m from the UK to and scheduled for TR on the 1st oct 07..just like chrissy said..
yes i needed my NHS number which i got from my GP surgery..if you need any more info then feel free to ask..there is quite abit of form filling once TR is scheduled but we are all here to help..
Emmie & the other ladies helped me and i still call on them for info,one of the main things is the 5hr time difference from the UK to NC..when makeing calls etc.. if you need me i spend most of my time in the waiting room part5..or the over 40s theard, good luck to you..

We see many patients come to Dr. Berger not only from the UK but from other countries also. With the information that is provided and the availability of the staff members, it makes the whole experience traveling from far distances a positive one.
